Home is Where the Health Is: CHPF’s Mother & Baby Care Mission

The CHPF – Mzazi na Mtoto Project Team at Chidongo💙
At the Community Health Promotion Foundation (CHPF), we believe that every mother and baby deserves a healthy start. That belief is the driving force behind our Mzazi na Mtoto Project, an initiative that is transforming access to maternal and newborn healthcare in the across our catchment areas. 💙

Through this project, we are taking Postnatal Care (PNC) services directly to mothers — right at their doorsteps. By reducing the distance to essential care, we are breaking down one of the biggest barriers that many families face: accessibility. For new mothers, especially those living in rural or underserved areas, visiting a health facility can be challenging due to lack of transport, time, or support. Our solution is simple — bring care to them.

What We Offer

Our Postnatal Care services are thoughtfully designed to prioritize the health, safety, and well-being of both mother and child. These services include follow-up visits after childbirth, breastfeeding support, monitoring for postpartum complications, newborn health checks, and guidance on infant nutrition and care. All of this is delivered with compassion, professionalism, and cultural sensitivity by our trained community health workers and healthcare professionals.

Rewarding Healthy Choices

To encourage regular and safe antenatal care, mothers who attend at least six antenatal care (ANC) visits during pregnancy receive a special gift of baby essentials. These packages may include items such as baby clothes, blankets, soap, diapers, and more — simple but meaningful items that help new mothers care for their babies. It’s our way of saying thank you and reinforcing the importance of consistent prenatal care.

Why It Matters

Every life matters. Every mother deserves to feel supported, and every baby deserves a healthy beginning. Postnatal care is a crucial — yet often overlooked — aspect of maternal and child health. With timely interventions, education, and emotional support, we can prevent complications, detect health issues early, and build healthier communities.

A Healthier Tomorrow Starts Today

At CHPF, we’re not just delivering services — we’re nurturing trust, building resilience, and creating lasting change. The Mzazi na Mtoto Project is more than a healthcare initiative; it’s a commitment to empowering families and strengthening the fabric of our communities, one household at a time.

Together, we can ensure healthier beginnings and a brighter future for every mother and child.
