About the program

The Young Families Program is a health-care service for adolescent mothers and their children. Our program acts as the medical home for both the mother and infant and will follow the family until the child/children reaches two to three years of age. The services provided include comprehensive health care, management of acute/chronic illness, nutrition or feeding concerns, parenting education and support, sexual health, psychosocial/mental health support, and co-ordination of care with community partners.

  • Routinely examine registered women, men and children
  • Identify and refer women, men or children with abnormalities or chronic illnesses
  • Give health education to women and men with children

Milestones

    • Start-up of kitchen gardens in the community
    • Providing health education to men and women
    • Gifting and after birth counselling
    • Screening to men, women and their children during their CWC
    • Administering preventive medications like Vitamin A, albendazole
    • Referral to level 2
    • Bringing services closer to the community, that is during the outreaches

CHPF-Young Families Education

Parenting education promotes the use of positive parenting practices, such as using positive language, planned discipline, and family routines. It also encourages nurturing behaviour and increases parents’ knowledge of child development and communication styles. 

We intent to have a well informed community on health and all spheres of life through civic education to young families training. the impact of the CHPF program n young families has yielded a positive mark in our catchment areas. 

The program is also designed to bring young mothers together and enable them realize their contribution to the society. however CHPF has a psychosocial support plan to help hem manage the stress that result from early pregnancies. North Coat Medical Training College students are as well included in the program to enable them gain experience on tackling critical maternal cases.
