Home-based care (HBC) is the provision of healthcare services directly in a person’s home to promote health and well-being. It aims to shift care from hospitals and other institutional settings to the community, allowing individuals to remain in their homes while receiving necessary medical support. HBHC can be provided by trained professionals like nurses, therapists, or home health aides, and it often includes a range of services such as medication management, wound care, physical therapy, and emotional support.

Key aspects of home-based care:

  • Focus on individuals and families:

HBC recognizes that health issues can affect entire families, and therefore, it may extend services to support the needs of caregivers and family members.

  • Community-based approach:

It emphasizes the role of community-based organizations and community health workers in delivering care and support.

  • Flexibility and personalization:

HBC allows for tailored care plans that address the specific needs and preferences of each individual.

  • Cost-effectiveness:

By reducing the need for hospital stays and other institutional settings, HBC can help lower healthcare costs.

  • Improved outcomes:
  • HBHC can lead to better health outcomes, increased patient satisfaction, and a greater sense of autonomy for individuals.
